Who determines what is P.C. and what is not?

The courts, politicians, the people, the media? Is there a democratic vote on each PC issue that all Americans get to vote in to determine what is PC or not? Or is there maybe some secret council of "special people" who decide and if so what gives them the right to determine what is PC or not? And the name: Political Correctness. Isn't that an oxymoron?
